Volunteer CORI submission
This is a secure page for uploading your CORI form and photo ID to the Blackstone Valley Emergency Shelter administrator. All uploads from this page are protected by end-to-end encryption.
Suggestion: If you don't have a scanner, use this QR code to access this page using your phone or tablet so that you can use its camera to upload your completed form.
Step 1
Download the blank CORI form and print both pages.
Step 2
Sign and date Page 1 of the CORI form. On Page 2, complete fields under Subject Information and Current Address. Please DO NOT sign Page 2.
Step 3
Provide your name and email address below, then use the "Add document(s)" button to attach each page of the CORI form, as well as your photo ID. Once both pages of the CORI form and your photo ID have been added, click the Submit button.
