Volunteer CORI form submission
Step 1
Download the blank CORI form and print both pages.
Step 2
Sign and date Page 1 of the CORI form. On Page 2, complete fields under Subject Information and Current Address. Please DO NOT sign Page 2.
Step 3
Provide your name and email address below, then use the "Upload" button to upload each page of the CORI form, as well as a picture of your photo ID. Once both pages of the CORI form and your photo ID have been added, click the Submit button.
Suggestion: If you don't have a scanner, access this page using your phone or tablet so that you can use its camera to upload your completed form.
